BSA Medical Form

Standard BSA Form  (Note: The BSA now has a single medical form which is used for both boys and adults attending official BSA events. It consists of three parts, A, B and C. Parts A and C are to be completed by the Scout or adult. Part B is required for Summer Camp and other long-term official BSA events--other than Seabase, discussed below--and must be completed and signed by your personal doctor.)

Seabase   (Note: The BSA National Seabase has its own special medical form--the same for both boys and adults--which must be used; the standard forms are not accepted.)
