BSA Medical Forms

Class 1 & 2    (Note: This form is required for Scouts attending Camp Friedlander or another official BSA camp. Parents must fill out and sign the front page, or the "Class 1" part every year. The back page, or the "Class 2" part, must be signed by a physician once every three years.)

Class 3    (Note: This form is required for adults attending Camp Friedlander or another official BSA camp. The portion requiring a doctor's signature is good for three years for Scouts and adults under 40 years of age. Adults over 40 years of age must get a doctor's signature every year.)

Seabase   (Note: The BSA National Seabase has their own special medical form--the same for both boys and adults--which must be used; the standard Class 1, 2 and 3 forms are not accepted.)
